We feel very strongly that each member of your family over the age of 18 should have both of these documents properly filed at all times.

The laws governing Advance Directives and the types available vary from state to state. To make things easier, Health Plan NavigatorTM has assembled an Advance Directives Kit for each state, and a booklet describing the choices that are available to you, and the discussions you need to have with your doctor and your family to make decision-making at a difficult time easier for everyone. We also suggest a variety of places these documents should be filed.
