  If you do not
Having an Advance Directive properly filed is one of the most important end-of-life documents you can compose. Without it, your physician could make decisions you wouldn't want. Family members may feel forced to make choices for which they don't want responsibility. Worse, relatives may actually dispute various treatments, wasting valuable time and energy, or causing family rifts that may never be healed.

Because Health Plan NavigatorTM also provides a wallet-sized document, you can also be confident that, in the event of an accident or sudden illness, your wishes will be the ones followed.

Making an Advance Directive is a really a gift you give to yourself.
