  All health plans now have an appeals and grievance system for claims or procedures that are denied. Call your insurance company and find out the guidelines, and follow them to the letter. Pay special attention to deadlines. Get your doctor to write on your behalf, and gather any other information that might be helpful. Make as strong a case as you can, and be prepared to submit it again if you are turned down. Insurance companies usually have several layers of appeals you can go through.
